Offering essential medical ID jewelry to help patients communicate critical health information in emergencies, providing peace of mind.
Blood Pressure Monitoring Device Program
Hypertension is the second most prevalent co morbid condition in CKD nationally. In the KFCP 28 county service area it is the Primary co morbid disease process. Sponsored by a grant from Highmark, Inc. since 2020, this program provides devices to patients in community and nephrology clinics as well as dialysis and transplant centers. Providers may also refer at risk individuals.

Patient Emergency Assistance Fund
The Patient Emergency Assistance Fund offers critical financial support to kidney patients facing emergency hardships. This fund helps patients to focus on their health and recovery during challenging times. The availability of funds is variable and depends largely on donations and grants.

Kidney Updates with Hershey Medical Center and Innovations and Trends with Penn Medicine Providing healthcare professionals, patients, families, and community members with the latest research, best practices, and educational resources to enhance kidney care.

Organ Donor Awareness
Raising awareness and advocating for increased living organ donation to reduce wait times for life-saving kidney transplants.
