Getting the best possible health care requires that all people involved work together - especially the one receiving care! People who have chronic kidney disease have a much better outcome if they are active members of the team.
MEDICAL TEAM encompasses physicians as well as what are known as physician extenders, including certified physician assistants and nurse practitioners. These specialists are well-trained members of the medical community - qualified to provide supplemental support for physicians.
CAREGIVERS should provide support to those physically unable to carry out certain tasks but allow loved ones the freedom to direct their own destiny. Supporting a chronically ill patient can be a difficult path to navigate.
CERTIFIED PARTNERS are highly trained volunteers that are developed through a mentorship program called Patient and Family Partner Program (PFPP). Certified Partners offer emotional support to patients, caregivers and donors struggling with the many challenges associated with CKD.
SUPPORT GROUPS are where patients and caregivers can gather with other people going through a similar experience. The groups provide each other with emotional support and discuss important topics like kidney transplantation.
